NV4500 vs NV3500

Should I hold out for a NV4500 or is my worry about the NV3500 overblown?

You mentioned gas mileage with the 4.3... you'll get better mileage with the NV3500, Less rotating mass. And there is no way you can blow it up with a 4.3.

NV4500 or SM465 is WAY overkill for anything but a built SBC or factory+ BBC.

Behind a 4.3, the nv3500 would be my preference. It shifts easier, and with less rotating mass, it theoretically would take less energy to turn. The nv3500 is a stout transmission when not subjected to shock loads or excessive power levels. Don't turbo the 4.3, and it will be fine!
Still turbo the 4.3! lol

A non abused nv3500 is perfectly fine for half ton truck use. The weakest part of a nv3500 is the shift forks. They are made from aluminum with a wear resistant coating. I had the nv3500 in my gmt800 rebuilt because teenager po liked to shift way too hard. The gears were still in perfect shape.
